On every users profile there is a place for the mods or admin to add notes about that user. Often there are no notes. Sometimes there are notes about how helpful a particular person is or that they were calm when under duress, etc. There could also be notes about moderation such as when someone was warned or given a temp ban.

Or the particular mod has a weird obsession with bad cookies. And some mods like to put bad jokes into notes so that other mods will think that a person is actually helpful.
