But chapter 458, when Ishida first showed up, didn't Tsukishima attack him? The panels aren't very clear, but he was standing behind him for the surprise attack.

I wasn't talking about that part! >_< Yes, it's already been clearly revealed that when Ishida was de-armed early in the arc, it was Ginjo that did it, even though he attacked along with Shu. The incident I am talking about is much later, whenever Ginjo reveals his evil-ness. Both Ichigo and Ishida are attacked. Ginjo gets Ichigo, and Shu gets Ishida! Let me see if I can nail down the chapter... Okay, it's chapter 458, pages 15 and 16, assuming it was numbered accurately on that site and corresponds to what you guys have. It's a little hard to tell, but I think that's what happened.
And yes, I agree that Ginjo is probably talking about SS or someone involved with them, but even so, nothing can excuse how he has gone about things. I can't wait for Wednesday to hear what he says though, still!
And checking back at those earlier chapters, I remember that Moe gave Ginjo some trouble when he was on Ichigo's side, which Ginjo probably remembers, which is why he wants that guy dead, not only because of the Jackpot Knuckle troublesome power, but probably also because of Ginjo's pettiness.
Somebody else made an interesting comment as well. If Fullbringers' power disappear when they die, is that why Ginjo hasn't killed Ichigo yet, because he doesn't want to lose his Ichigo-power-up? Presumably all the still living Xcutioners would lose theirs as well.

He's cute, but that Yukio is a little turd! Saying he was hoping Riruka was dead. Hmph, he's complaining about hands and feet? Toshiro should have threatened to freeze his juevos! 
Hopefully she's alive in Rukia's body (despite her disturbing past and what she did to Rukia, I still can't bring myself to hate Riruka), but since Byakuya said he detected no trace of her power, and we know what Ginjo says later about Fullbringers dying... T__T
I've already told you guys that Tsukishima won't die.
Riruka, possessing Rukia, will turn Tsukishima into a stuffed animal, that Ishida the sewing master, having been cut by Shu, will then repair, and Shu will be saved!
After all, Riruka said that Shu made the Love Gun, and when there was an explanation given that Shu could even make himself a lover with his power, a picture of Riruka was shown. Hmmm... Of course, this is totally silly, but I wouldn't put anything past Kubo! And Ishida is bound to get out of that dimension some time, and given the timeframe of manga/anime, I won't be surprised if the battle manages to finish within ten minutes (in-universe).
We all wait with baited breath who Ginjo will say Ichigo should be fighting. CONSARN YOUR CLIFFHANGERS, KUBOOOOOO!
One last thing, Aizen may have been a megalomaniacal loon (I LOVE that phrase!) that committed endless atrocities and inflicted untold suffering, but Ginjo went way too far, for some reason, making it personal with Ichigo's loved ones.
So yes, Ichigo still has plenty of reason to continue fighting, despite Ginjo's latest mind game. NOTHING Ginjo says next week will excuse his actions, but I still want to know, and I hope we also learn about Ginjo's past, as well as Shu's and hopefully Isshin's, the story behind Substitutes and whatever horrible thing Soul Society did this time!
