In the manga, Inuyasha was more concerned about his inability to protect Kagome than his own vulnerability.
Until Kagome mentioned it to him, Inuyasha never realized that what had happened was a transformation into full demon. He never realized that he had looked that scary. When Kagome said that she thought he had forgotten her while in demon form, he first realized Miroku's warning may be right, but quickly denied it. His responding declaration was something of a declaration of affection, although he didn't seem to realize he had done that. "I could never forget you, Kagome." But it did concern him that his heart, his soul, may have changed during the transformation.

It's cool how he was able to admit that, to himself at least, that he can't forget her, though with Inuyasha who knows he may have realized he had said that, he has no problem saying sentimental things in his mind, his only problem is forgetting 
His first transformation and he didn't remember it, he can try to deny it, but of course there is some fear, you can actually tell he's worried, wen Kagome said "I thought you would forget about me.", Inuyasha's response followed, "I could never forget you, Kagome." He does fear but like always he denies it, (Since it shows weakness).
